Welcome to online registration for our PHCA/CALM 61st Annual Convention and Trade Show. You may use this form for fast, online registration.

Hotel Reservation: PHCA/CALM has a room block with the Valley Forge Casino Resort. To receive the special rate of $119/night plus 6% sales tax and 2% occupancy tax, please make your reservation before October 21, 2012. Mention the PHCA/CALM Convention room block when making your overnight arrangements.

Attendee Preferences Questionnaire. As part of your online registration, you will be asked to complete a questionnaire indicating which days you plan to attend the conference. You will also be asked whether you require any special services, such as special dietary needs.
Cancellation Policy: Cancellations made at least 7 business days prior to the start of the convention will receive a full refund less a $50 administrative fee. However, no refunds will be made on cancellation requests received on or after Monday, November 5. To cancel, please call (717) 221-7932 no later than November 5.
